We have two versions of the Paper Survey available on-line. |
- Acrobat PDF version of the paper survey. It does not perfectly reproduce fonts or page layout, but loads quickly.
- A scanned version of the paper survey used. It is a larger file, and takes longer to download.
| The Web Version of the survey was only available to students entering a 7-digit identification number. To see the survey, and to try completing it, enter the ID number 111, 222, 333, or up to 999. |
